Se presenta un caso de dilatación aneurismática de aorta abdominal e ilíacas primitivas de grandes dimensiones con cuello difícil, angulado a noventa grados a poco del nacimiento de las arterias renales. Esta condición se presenta en un paciente hipertenso con Insuficiencia Respiratoria restrictiva severa que debuta con embolia isquémica en miembro inferior izquierdo. Se plantea la discusión sobre la estrategia quirúrgica para la resolución, que finalmente se abordó por vía endovascular.
This article presents the case of a large abdominal aorta and primitive iliac aneurysmatic dilatation with difficult neck and an angle of 90º degrees located near the beginning of the renal arteries. This condition is present in a hypertensive and solitary kidney patient with severe restrictive respiratory failure that begins with ischemic embolism of the left lower member. A discussion on the surgical strategy to resolve the case is presented. Finally, the condition was treated by means of an endovascular route.
