INTRODUCTION:

Gallbladder volvulus (GV) is caused by the rotation of the gallbladder on its mesentery along the axis of the cystic duct and artery, compromising its vascular supply, with certain predisposing anatomical characteristics. It is a rare pathology and therefore rarely reported, despite which several reviews have been carried out.

ClLINICAL CASE:

Elderly woman, thin, 77 years old, previously healthy, who suddenly presented with symptoms of complicated acute cholecystitis, laboratory tests with inflammatory parameters and compatible Computed Axial Tomography. An emergency laparoscopic cholecystectomy was performed, finding a GB with signs of ischemia, with a satisfactory postoperative evolution. The biopsy confirmed acalculous gallbladder necrosis.

DISCUSSION:

GB is a pathology that can simulate acute cholecystitis; its diagnosis is not usually considered as an intraoperative finding. In the patient we report, the behavior was appropriate. This entity has a good prognosis when the diagnosis is made in time. The treatment of choice is laparoscopic cholecystectomy.

OBJECTIVE:

To report the first GB resulting laparoscopically in our country.
