Respuesta clínica de sustitución hormonal en pacientes con déficit de Hormona de Crecimiento

El tratamiento de los pacientes con talla corta por deficiencia de hormona de crecimiento (DHC) se realiza en base al reemplazo hormonal, cuyos efectos son beneficiosos desde el punto de vista clínico, psicológico y social.

Objetivo:

Describir la respuesta clínica, a la terapia de reemplazo hormonal, de los pacientes con deficiencia aislada de hormona de crecimiento, atendidos en el servicio de endocrinología del Instituto Nacional de Salud del Niño.

Métodos:

Se realizó un estudio descriptivo, retrospectivo y longitudinal. Se analizó la respuesta a la terapia de reemplazo con hormona de crecimiento (RHC) en 10 pacientes con deficiencia aislada de hormona de crecimiento.

Resultados:

La edad al diagnóstico fue de 4.54 ± 2.4 años siendo la edad al inicio del reemplazo con hormona de crecimiento de 5.9 ± 2.8 años. La velocidad de crecimiento pretratamiento fue de 4.5 cm/año y aumentó desde los seis primeros meses de tratamiento, alcanzando 11.6 cm/año al final del primer año de RHC. El SDS de la talla pretratamiento fue de -4.19± 0.74 Y de -2.87±0.73 al final del primer año de RHC. La ganancia de talla según la proyección de talla adulta por el método de Bayley-Pinneau fue de 8.2 cm el primer año y de 10 cm a los 2 años de tratamiento. No se encontraron alteraciones en los niveles de glicemia y ni en el perfil lipídico.

Conclusiones:

El tratamiento con Hormona de crecimiento fue efectivo en aumentar la talla de los pacientes con deficiencia aislada de hormona de crecimiento

The treatment of the patients with short stature caused by growth hormone deficiency (GHD) is realized on the basis of the hormonal replacement which has a positive impact on the clinical, psychological and social setting.

Objective:

To describe the clinical response, to the therapy of hormonal replacement, of the patients with isolated growth honnone deficiency, attended in the Peruvian Endocrinology Unit at Instituto Nacional de Salud del Niño.

Methods:

This is a descriptive, retrospective and longitudinal study. We reviewed the clinical response to the therapy of hormonal replacement of 10 patients with growth honnone deficiency.

Results:

The age of diagnosis was 4.54 ± 2.4 years; the age at the beginning of the honnone replacement was 5.9 ± 2.8 years. The growth velocity was 4.5cmlyear pretreatment and increased since the 6th month of treatment, reaching 11.6 cm /year at the end of the first year of honnone replacement. The score of the standard deviation (SDS) of the pretreatment height was -4.19 ± 0.74 and -2.87±0.73 at the end of the fírst year of treatment. Height gain according to the Bayley-Pinneau's method was 8.2 cm after one year and 10 cm after 2 years of treatment. We did not find any alterations in the glucose levels and lipids profile.

Conclusions:

The treatment with the Growth honnone was effective in increasing the height in the patients with isolated of hormone of growth
